가까운 눈밑은 부드러운 지방층이, 먼 눈밑(중안면, 앞광대)는 골격이, 더 멀리는 안륜근 밑 지방층이 동심원처럼 위치합니다. 중년 이후 경계가 뚜렷해지면서 불룩한 지방층과 중안면 함몰로 인한 하이라이트와 쉐도우가 뚜렷해지는 현상이 다크서클입니다. 나이가 들면서 안륜근 밑 지방층이 더 불룩해지고 내려가면서 다크써클과 팔자주름을 더욱 깊게 만듭니다.
The lower eyelid area has different layers arranged like rings: soft fat close to the eye, bone support in the midface, and deeper fat below. As we age, the borders between these layers become more visible. This causes bulging under the eyes and hollowing in the midface, creating shadows. This contrast of light and shadow appears as dark circles and eye bags.

주름제거와 안검외반 Wrinkle excision & lid eversion (ectropion)
주름은 과잉 피부를 절제해 개선합니다. 가장 주의해야 할 부작용인 안검외반은 절제량이 지나치거나, 안륜근이 손상된 경우 일어납니다.
첫 하안검에서는 안륜근이 손상되었어도 크게 문제가 없다가 2차 수술에서 이전 손상되었던 안륜근이 약해져 한계치를 넘으면 안검외반이 일어날 수 있어 이전에 하안검성형을 받으셨던 분들은 꼭 알려주셔서 격막을 강화, 외안각 고정은 강하게, 피부 절제는 보수적으로 시행해야 합니다.

눈밑과 인중사이 앞광대를 중안면부라고 합니다. 눈밑처짐에 추가로 중안면 안륜근 밑 지방층 SOOF이 하방으로 이동해 팔자주름이 깊어지고, 함몰되어 그림자가 생깁니다. 처진 팔자주름 위 지방층의 뒷벽을 엔도타인을 이용해 올려서 고정해줍니다.
The midface spans the area between the lower eyelid and upper lip. Aging causes the SOOF to descend, creating hollows, deeper nasolabial folds, and shadows. Midface lifting restores a youthful contour by elevating the SOOF and anchoring it to the cheek bone.
접근 Approach
- 하안검 성형 방식에서 박리를 좀더 진행해 SOOF지방층 뒷면을 중안면용 엔도타인으로 고정해주는 방식
- 관자놀이와 입안절개 각 1.5cm정도를 통해 중안면용 엔도타인으로 고정하는 방식
Both approaches are performed under local anesthesia. The SOOF can be elevated through a subciliary incision (Lower bleph approach) and secured to the cheek bone with a midface Endotine, or alternatively, through a 1.5cm temporal and oral approach.
중안면 엔도타인 Midface Endotine
중안면 조직을 골격에 고정하는 엔도타인은 작은 동전크기에 조직을 고정하는 돌기가 조직을 잡아주며, 조직의 밑면에서 끌어 올리므로 표면에서 실리프팅과 같은 굴곡이 발생하지 않습니다. 10개월 후 폴리락틱산PLA 성분이 흡수되며 유착을 만들고 사라집니다.
The midface Endotine is a small, coin-sized bioabsorbable implant with multiple micro-tines that secure tissue from beneath, avoiding surface dimpling often seen with thread lifting. It is composed of poly-L-lactic acid (PLA) and is gradually absorbed over about 10 months while facilitating long-term tissue adhesion.
